Registration
All players registering for any Ballistic program are required to sign-up online at this website.
Team Placement
Teams will be arranged in accordance with the following:
Girls will be placed based on her grade at time of play (if summer season, then her rising grade) on either a Middle School and High School team.
Boys will be placed based on players’ birth dates per USLacrosse age grouping. All players are required to provide a copy of his/her birth certificate upon request and have it available for inspection by tournament directors at tournaments/games.
Eligible players must be between the ages of 10-18, as of September 1st occuring prior to the season of regitration, and cannot be simultaneously on any other rec or travel team roster.

USLacrosse Membership
All players must be a member of US Lacrosse with a membership that is valid thru May 1st for Spring, June 30th for Summer, and December 31st for Fall. Register for US Lacrosse at www.uslacrosse.org. The cost for a youth membership is $25.00 or $35 per year depending on age, payable to USLacrosse. This fee is in addition to the Ballistic registration fee. All players are required to provide a copy of his/her US Lacrosse card at or before first practice/tryouts.
Required Forms
The following forms must be turned into Ballistic Sports Group, Inc., at or before the first practice/tryouts.
**No player will be allowed to participate without the required forms and a USLacrosse membership card, valid through the end of season, on file with Ballistic Lacrosse at or prior to first day of tryouts and/or practice.
Payment and Cancellation/Refund Policy
Payment is due upon registration. A payment plan is offered during registration. Payments can be made by credit or debit card (Visa, MasterCard, AmericanExpress or DiscoverCard), or by mailing a check to: Ballistic Sports Group, Inc., PO Box 63, Alva, FL 33920. Ballistic Sports Group, Inc., does not issue refunds. However, in the event a player is seriously injured and can not play a partial refund may be offered at the soul discretion of Ballistic Sports Group, Inc.. Once a player is assigned to a team, event, and/or tournament, he/she is not entitled to a refund for any reason. Ballistic Sports Group, Inc., is a non-profit organization and does its best to deliver the best youth lacrosse experience possible for all its players, but cannot make any guarantees as to practice and game schedules, playing time, coaching staff, training or game experience, or other policies and procedures. Once a player registers for one of Ballistic Sports Group’s programs, the organization incurs expenses to prepare for that player’s participation.
