Terms and Conditions: By checking the box you hereby: a) certify that you understand payment is due at time of registration. If paying by check, payment will be received no later than 5 days (excluding Sundays) from date of registration; b) certify that the registering player is a member of USALacrosse valid through the last day of the current season, is the appropriate age for his team for the current season per USALacrosse standards, and is not on any other team roster. You certify that all personal information submitted is true and correct; that all required forms (available on this website under "Requirements") will be submitted to Ballistic Sports Group, Inc., either with this registration or at or before the first team practice; and a copy of player's birth certificate will be available at all games for age verification, if requested. c) certify I understand that in attending any sport program and using the facilities do so at the participant’s own risk. Ballistic Sports Group, Inc., its representatives, Lee County Parks and Recreation, The School Board of Lee County, and any and all other venues used by Ballistic Sports Group, Inc., and their agents, shall not be held liable for any damages whatsoever arising from any personal injury or property loss or damage sustained by participants or family members on the premises. Participants and parents/guardians assume full responsibilities for all injuries and damages with may occur in or around any program on the premises and hereby fully and forever release, discharge and hold harmless Ballistic Sports Group, Inc., and all associated facilities and their owners, agents and employees from any claims, damages, rights of action, present or future, resulting from or arising out of any person’s participation in any program or use of its facilities. In addition, participant agrees to follow the rules and conduct set by Ballistic Sports Group, Inc.. Participant and parent/guardian understand that failure to comply with rules and regulations will result in suspension from participation. d) grant authority to Ballistic Sports Group, Inc., Lee County Parks and Recreation, and The School Board of Lee County, and any and all other venues used by Ballistic Sports Group, Inc., to render a judgment concerning medical assistance or illness during my absence. e) grant Ballistic Sports Group, Inc., the right to photograph and/or videotape me and participant and further to display, use and/or otherwise exploit my and/or participant’s name, face, likeness, voice, and appearance forever and throughout the world, in all media, whether now known or hereafter devised, throughout the universe in perpetuity (including, without limitation, in online webcasts, television, publicity, newspapers and magazines) and in all forms including, without limitation, digitizing images, whether for advertising, publicity or promotional purposes, including, without limitation, publication or event results and standings, without compensation, reservation or limitation. f) certify that you understand Ballistic Sports Group, Inc., does not issue refunds. However, in the event a player is seriously injured and cannot play a partial refund may be offered at the soul discretion of Ballistic Sports Group, Inc.. Once a player is assigned to a team event/tournament, he/she is not entitled to a refund for any reason. Ballistic Sports Group, Inc., is a non-profit organization and does its best to deliver the best youth lacrosse experience possible for all its players, but cannot make any guarantees as to practice and game schedules, playing time, coaching staff, training or game experience, or other policies and procedures. Once a player registers for one of Ballistic Sports Group's programs, the organization incurs expenses to prepare for that player's participation.
